Remember the fundraiser is M, T, W next week. You must bring your voucher with you.
Tuesday is "team dinner" night. I want to see everyone there that night!
If you need more vouchers come see Coach in P2.

Bring your friends and just tell them to bring cash so that you don't have to split up the check!

Posted Jan 14/10 - Documentation updates...

Most of you have turned in your documentation. If you have not, please make sure you provide me with copies of the following:
~Current Duval County Athletic Physical completed (can be found in main office)
~Copy of current insurance card
~Insurance verification form
~Copy of birth certificate
~Completed FHSAA consent form

NOTES:
~If you have documentation with another coach, you need to ask the coach for a copy of all documentation and give the copies to me.
~Your birth certificate must have the raised seal. You can take it to Coach Talley and she will make a copy.
~If you turned in a birth certificate last year, I am going to have the managers go through last year's documents to check for them. If I cannot find your old birth certificate, you will have to get another to me.
~All forms must be updated every year EXCEPT the birth certificate.

If you borrowed a black jersey last week OR still have your black uniform from last year, I need you to return it ASAP. I need to get an exact count to find out how many uniforms I need to order.

Please wash them before you return them.
