2013 YOUTH REGISTRATION

Complete the 2013 Sauk Prairie Youth Tackle Football Form & Info document found under the Handouts tab.You should keep pages 1-4 as it contains detailed information on the important dates,times, etc.. Pages 5-8  should be completed and turned into the Community Center.

• Fees: $75 affiliated/ $95.00 non affiliated
• Registration Deadline is May 17, 2013
• Register any time prior to May 17, 2013 at the Community Center.

All registration forms need to be mailed or delivered to the Sauk Prairie Community Recreation Department located at 730 Monroe Street, Sauk City 53583. Any  questions: please email or call 608-643-0520.

This fee includes the following:

• 2013 Sauk Prairie Youth Football Season
• Current 4^th & 5^th grade - summer sports performance training
• Current 6^th & 7^th  grade - Summer School Class 550 – Maxper4mance-Register for this through the Summer School Program.
• Sauk Prairie Football camp – all 4-8 grade player

 

 

2013 PARENT MEETING

Location: Grand avenue practice field
August 8th, 2013 at 6 PM

• A parent/guardian should try to attend
• Discuss our program
• Pick up game schedule and practice schedules- these are determined by your coach
• Meet your Coach
• Answer any questions you have
