Coach Kit

•   Coach/Manager Responsibilities - This document outlines the responsibilities of the Team Coach and the Team Manager.
•   Code of Conduct Forms - This file outlines the Conflict Resolution Procedure as well as the Code of Conduct for players, parents, and coaches.
•   Player Medical Information Sheet - This form is to be filled out by each player so any pertinent medical information is known by the team officials. This information is to be kept confidential.
•   Pitch Count 11U - Pitch Count Sheet to use until access to Pitch Count App has been given
