In order to stay in the wrestling athletic period, receive a locker, obtain practice gear, and work out before, during or after school, you need to have the following forms turned in to various offices.  Click here for all the information.

Everyone needs to turn in numbers 1-4.  Numbers 5-6 are for athletes new to SLHS Wrestling.  Expect at least a three day delay for your physical to clear if you wait until school starts to turn it in.

1. Completed Physical Packet.  Needs to be turned in to the trainer's office in the field house between the pool and main weight room. 2. $50 Participation Fee.  Make checks payable to "KISD Athletics".  Only one fee required per child per year no matter how many sports played. 
3. Signed Parents' Packet (signature page only).  Will be available the first day of school.
4. Current Utility Bill from June 2014 or later.  Feel free to black out any account information.  We just need a bill that matches the address used on all other forms.

6. If you have moved or transfered into the SLHS zone less than 365 days ago, you need to contact Coach Demarchi as soon as you can so we can start the lengthy process to get them legal.
