| Most of you have turned in your documentation. If you have not, please make sure you provide me with copies of the following: ~Current Duval County Athletic Physical completed (can be found in main office) ~Copy of current insurance card ~Insurance verification form ~Copy of birth certificate ~Completed FHSAA consent form NOTES: ~If you have documentation with another coach, you need to ask the coach for a copy of all documentation and give the copies to me. ~Your birth certificate must have the raised seal. You can take it to Coach Talley and she will make a copy. ~If you turned in a birth certificate last year, I am going to have the managers go through last year's documents to check for them. If I cannot find your old birth certificate, you will have to get another to me. ~All forms must be updated every year EXCEPT the birth certificate. |
